2. Foreman for hot work operation shall complete this form prior to commencement of the hot work. Employee performing hot work shall review this form and display in the area where work is being done.

Will the sprinkler system or alarm system be impaired?

When do you need a Fire Watch?
Flamable or combustible materials are within 35 feet and cannot be protected.
Slag/sparks are falling to lower level or behind a wall or obstruction when the slag/spark cannot be managed by operator.
Hotwork is being performed in an occupied building.
Hotwork is being performed in a structure protected by sprinklers, in which the fire protection devices are inactive.
